Download, print, and Sign Nomination Forms

Important Instructions (download form below):

1. Using the link below, download and print the nomination form (or email info@lisasubeck.com to request a hard copy by mail).
2. Sign your first and last name in the column marked "Signature of Electors." Please note that only one name may be written on each line.
3. Print your first and last name in the column marked "Printed Name of Electors."
4. Print your full residential street address (number and street) in the column marked "Residential Address." Please note that P.O. boxes are not acceptable.
5. Print the name of your municipality (city) in the column marked "Municipality of Residence." If the city used for mailing is different than your city of residence, please use your city of residence for voting purposes (ex. you may live in the City of Madison but have a Verona or Middleton mailing address).
6. Fill in the exact date on which you signed the nomination form under the column labeled "Date of Signing."
7. Please also collect the signatures of other household members or neighbors on the same form if you are so inclined.
8. Near the bottom of the form, under the heading, "Certification of Circulator," please print both your name and full address (number, street, municipality, and zip code). Note: You are the circulator of the form, so it is very important you complete the certification fully.
9. At the very bottom of the page, properly certify the nomination form by both signing and dating the certification portion of the paper. Note: The date you sign the bottom must not be prior to the dates of any signatures above on the form. Any signatures dated after the date of certification will be considered invalid, so please sign and certify only after signatures are collected.
10. Please mail your completed nomination form as soon as possible, and definitely by May 15, to Friends of Lisa Subeck, PO Box 44308, Madison, WI 53744. You do not need to fill the page with signatures - forms may be submitted with as many as 10 or as few as 1 signature as long as they are properly signed and certified.
